Susan Gray Wellness is dedicated to inspiring health and relaxation. A registered biodynamic craniosacral therapist and a licensed and nationally certified massage therapist, Susan’s integrative approach combines craniosacral therapy, therapeutic massage, guided imagery, breathing techniques and essential oils to help generate optimal well-being.

If you have any trouble downloading and printing the above forms, please contact my office. You may either pick them up, or contact me by phone or email if you would like them mailed to you, or fill them out at my office before your first visit.
